Tripod Beta: Adopting a visual, Barrier Based Approach to Incident Investigation & Analysis. In your organisation, controlling your key processes is of the essence. Incidents and breakdowns at any level will harm your continuity and have an adverse effect on your results. In addition, safe working conditions will ensure that your team is working with confidence, achieving higher productivity and the best possible results. Operational disturbances therefore require insight as to their origin.

Tripod Beta is a ‘reactive’ method for incident analysis leading to proactive process control measures. It does not just focus on the immediate, human, causes of the incident, but identifies the structural flaws that allowed the incident in question to occur. The Investigation framework clarifies unknowns and uncertainties and thus leads to insights. Underlying causes are identified and lead to well considered conclusions and management actions, strengthening the organisation’s sustainability as well as business and safety performance.

Proven Success

Scientific research at Leiden (NL) and Manchester (UK) Universities forms the backbone of the Tripod Theory. Many years of applied research and extensive practical use have made the matured Tripod theory the best solution for both proactive and reactive risk management. Many companies and other large organisations have reaped the benefits of using the Tripod methodology, namely a considerable, measurable reduction in incident rates and a firm grip on risks.

Tripod Beta: Practitioner training

Our Tripod Beta Training teaches participants the Tripod theory and approach, and how to investigate and analyse operational disturbances. In addition, the supporting software facilitates the analysis of, and reporting on operational disturbances by the investigation team. It helps to explicitly identify underlying causes and take corrective action, improving process and safety performance.

Training Set-Up

Participants will spend part of the time in groups to work on cases in order to facilitate a mix of theory and practice. Participants are encouraged to bring in their own ideas and ‘real life’ cases. The key objective is to expand participants’ skills to quickly and effectively apply the Tripod Beta methodology and software in practice.

Target audience

The target audience of this training consists of, though is not limited to: incident investigators, (Q) HSE, project, site, plant, operations, risk, maintenance, and integrity managers.


Participants will be provided with a trial license for the Investigator 3 software, which will remain valid for 30 days after activation.


Participation fee for the 4-day training is € 2380,- (VAT excluded), including training material, certificate of participation, theory assessment as part of the Practitioner accreditation, trial software access and lunches.

Training Program Outline

Day 1: Introduction, Incident investigation process

  • Positioning of Tripod methodology in a broader safety management system context.
  • Barrier based risk management vs. barrier based incident management.
  • Understanding incident process & characteristics.
  • Effective incident investigation process including appropriate Terms of Reference.

Day 2: Tripod Beta methodology, incident analysis

  • Theory behind the Tripod model.
  • Introduction of key Tripod Beta elements.
  • Use Tripod Beta elements to construct accurate event, object, agent (EOA) diagrams (analysis).
  • Use Tripod Beta theory to complete the investigation plan by identifying appropriate barriers and collecting relevant information.
  • Understand investigation ethics based upon human behaviour and the human behaviour model.
  • Basic Risk Factors.

Day 3: Cases: Tripod Beta Incident Investigation & Analysis

  • Application of Tripod Beta in practical (own) real life cases and exercises.
  • (Optional:) Application of Investigator 3 software.

Day 4: More cases, Follow up & communication, Examination

  • Tripod Beta Practitioner examination.
  • Practical cases, exercises and presentation of case conclusions and recommendations.
  • Effective follow up and management communication.


Upon completion of the Tripod Beta Practitioner Training, attendees can opt to go through a formal Tripod Accreditation process as facilitated through the ‘Stichting Tripod Foundation’ (
