If you are planning on submitting samples to Actlabs, theinformation in this document will assist you during theentire process. We understand that high quality analysisand fast turnaround time are essential for all projects andthe information contained in this document will helpachieve this goal.3.1.A signed agreement or contract has beencompleted and approved by Actlabs. Paymentwill be issued after the invoice has beenreceived.4. Credit has been established with Actlabs (referto Actlabs' Credit Application Form). Paymentwill be issued after the invoice has beenreceived.If credit has not been established with Actlabs,payment must be received prior to releasing results. Ifyou require a proforma invoice in order to obtain apurchase order, specify; the type of samples, if yourequire sample preparation, number of samples,analysis required, and whether you require yoursamples returned or disposed after analysis.When submitting samples to Actlabs, we requirepaperwork detailing all of the information of yourrequest. Filling out a Request for Analysis Form willprovide all of the required information to process yoursamples and must accompany your samples. Werequire the following information for every batch ofsamples that are submitted:Complete address and name of the personsubmitting the samples and who will bereceiving the results and invoice (includingemails and fax)Submittal PaperworkCustomer ServiceContract and Billing QuestionsYou can rely on our responsive and knowledgeableSales and Customer Service team to help you with allof your needs. Heavy duty plastic samplebags, cloth sample bags, and soil envelopes areavailable at cost.Waybill instructions for International shipmentsPoorly labeled or packaged samples, samples havingincomplete or no submission sheets may not beprocessed until adequate written instructions arereceived from the client. Turnaround time will beimproved by proper packaging of your shipment. Seebelow for efficiency in shipping samples:1. Submit paperwork by e-mail prior to shippingsamples by providing an accurate list of allsample numbers, preferably with an excel file.Include a copy of the paperwork inside the bagor box of samples.2. Request for Analysis Form should indicatesamples that will not prepped and thosesamples that require prep i.e. Pulp, or CrushedRock that will only require to be pulverized etc.This will ensure that prep charges are appliedto only those samples being prepped and willminimize changes to our system afterwards.2. Is the person or company paying for theresults different then the company receivingthe results or submitting the samples? If so,has credit been established with Actlabs forthis different company? If not, please refer tocontract and billing questions.3.4.5.6.7.8.9.Once you've determined the analysis that you require,how you'll pay for the analysis, and filled out thenecessary paperwork, you're now ready to submityour samples to Actlabs.Customer reference to any project number,quotation, proforma invoice, or purchaseorder.Sample Disposal InformationNumber of samples and names of samplesType of samples and whether they needsample preparation (preparation code)Package Code for the analytical tests requiredCustomer AuthorizationIf you are unsure whether your samples aresuitable for testing upon receipt and requiresample preparation (for example, a rock wassubmitted and needs to be crushed and milledfor analysis), there is a check box regardingsample preparation charges: Contact me ifsample preparation is required OR I authorizeany required sample preparation charges.Selecting the latter option will minimize delayin the process. If you would like to verifywhether your samples are suitable for analysisas is, please contactcustomerservice@actlabs.com prior tosubmitting your samples.To improve turnaround time, the above informationcan be submitted electronically along with an accuratesample list (.doc, .xls, .csv) for faster processing ofyour sample batch. For samples submitted toAncaster, email the forms to:samplereception@actlabs.com. For other Actlabsfacilities, the email address can be found online orpage 3 of the Schedule of Fees and Services.If you need to ensure proper chain-of-custodydocumentation, use our Chain-of-Custody Recordinstead of the Request for Analysis and submit thecompleted form with your sample list.Shipping SamplesOnce the samples are received and all information isclearly identified, your sample batch will be given awork order number and a confirmation email will besent to you indicating the receipt of your samples. Ifthere are any discrepancies between the paperworkthat is submitted and the samples that are received,you will be contacted and your samples placed on hold,which will delay processing your order.In an effort to reduce our impact on the environment,all reports and invoices are transmitted electronically ina PDF format and/or as an excel file at the emailaddress you specified in your request for Analysis formsent together with your samples. If required, hardcopies of reports and/or invoices are available uponrequest. Quality Control data is provided with everyreport. If your require a customized reportingtemplate, please inquire internally withcustomerservice@actlabs.comInserting CRM's into your sample batches helps meetmeasurement needs by providing a known, wellcharacterized entity to compare and evaluatelaboratory analysis results. Actlabs can provideguidance in the proper selection of CRM's based onyour project needs. CRM's are commercially availablefrom the following sources:AMIS (South Africa)CANMET (Canadian Government)CDN Resource Labs(Canada)Geostats (Australia)OREAS (Australia and Analytical SolutionsCanada)Rocklabs (New Zealand)SARM (South Africa)Actlabs can also prepare customized StandardReference Material (SRM) for your projects and haveany suggested elements or group of elements certified,for more information contact crm@actlabs.comGenerally accepted results are those results lyingwithin +/- three standard deviations on either side ofthe certified values on the bottles rather than +/- 2standard deviations listed on the bottle. Actlabs has apresentation by Lynda Bloom (Analytical Solutions) andMaureen Leaver (Canmet), which can be provided toexplain the rational for this. This range of acceptabilityis called Control Limits. The control limits applicationsare set by the client.Reporting ResultsCertified Reference Material (CRM)???????3. Samples must be neatly in order in shippingcontainers. If using pop top vials or glassbottles ensure that they are taped shut andprotected from breakage. Sealed plastic bagsare best for shipping pulps. Additional sortingcharges will be incurred if chaotic shipmentsare received.4. Soil or vegetation samples from outsideCanada should be sealed in plastic 20 litrepails with tight sealing lids (or equivalent leakproof container) for shipping to avoid beingstopped by customs if they are leaking.Actlabs does have CFIA import permits for allcountries. We can provide this to you and youcan attach to shipment for seamless customsclearance.5. Ensure all samples are clearly identified withsample numbers. This can be accomplishedwith waterproof ink on the sample bags orwith sample tags in the sample bags.6. When shipping multiple rice bags or boxes,ensure the first rice bag/box in the ordercontains any paper work and that it is flaggedwith a different color flagging tape (for ricebags).7. Number rice bags in order, example Bag 1 -sample 60005-60010, Bag 2 sample 60011-60015….etc Ensure that both sample ID's andRice Bag numbers are in a visible location.8. When palletizing a shipment, pack the palletby putting the last bag in the order on firstand the first bag of the order on last. This willmake the sorting of the samples upon receiptby the lab more efficient and less timeconsuming as there will be no need to sort ricebags as they will already be in order.9. When submitting samples for Precious Metalsby Fire Assay, submit samples in batches of 35or a multiple thereof. 35 client samples will be1 full rack of 42 (including Actlabs QC) in FireAssay as client samples are fired individually.10. If there are multiple batches in a shipment,mark batches with separate color flaggingtape or separate color spray paint in a visiblelocation on rice bags. If submitting multiplebatches in one box, include all paperwork inthe same box.
