Anti-citrullinated protein antibodies (ACPA) are autoantibodies that are usually present in majority of patients which have rheumatoid arthritis (RA) and are directed against proteins and peptides that are citrullinated. The first members of this autoantibody family were discovered in the 1970s, these were anti-perinuclear factor and anti-keratin antibodies. However, later studies have demonstrated that autoantibodies from RA patients have the ability to react with a number of different citrullinated antigens such as vimentin, fibrinogen and deiminated epstein-barr virus nuclear antigen 1.

The primary diagnosis of RA in serological support and clinical manifestations has so far been restricted to the measurement of autoantibodies against rheumatoid factor (RF). This only has a moderate specificity of about 70% for RA, however, several recent studies have discovered that antibodies against citrullinated arginine residues which is present in filament proteins is found to occur in RF negative patients. Human ACPA ELISA assay displays high sensitivity and specificity for auto-antibodies against citrullinated vimentin. This is a citrullinated protein which have been identified to be in rheumatoid synovial tissue of RA patients. Also, the titer of antibodies against vimentin in RA patients shows a direct correlation with the disease activity score.

Human ACPA ELISA kit is a method intended for analysing in vitro quantitative levels of IgG antibodies against citrullinated protein (anti-citrullinated protein antibodies, ACPA) in human plasma and serum. This assay has a minimum analytical sensitivity limit of 1.0 U/ml.
