The Hazard Public Web Form enables organizations to have all stakeholders report hazards from any location. Stakeholders simply go to the web to report the hazard. No login, username or password is required. The hazard data is automatically transferred to the IndustrySafe Software, and IndustrySafe users are automatically notified by email of the Hazard Report.

IndustrySafe System Administrators can also determine required fields on the Hazard Reporting Form. The Hazard Reporting Form cannot be successfully submitted without completing all required fields.
Once a hazard is complete via the public form, stakeholders view a confirmation email; can upload attachments; and print a paper copy of their hazard report. All data is automatically transferred to the IndustrySafe Software for more detailed follow up, analysis, and analytics by key team members including environmental, health, and safety professionals. Key safety managers and supervisors can review the hazards submitted, evaluate hazard controls, or develop detailed corrective action plans for hazard resolution.
