Medgate - Version GX2 -Questionnaires

This module will enable you to:

• Create questionnaires for any purpose; audit/inspection checklists, incident witness interviews, training quizzes, risk assessments, equipment inventory, etc.
• Track environmental data that isn’t logically tracked elsewhere in the application
• Attach questionnaire responses to any other record in the application
• Gather information from across your workforce using a simple online questionnaire link
• Analyze questionnaire data using Medgate’s Business Intelligence tools

The questionnaire module offers the following features:

• Creating & Using Questionnaires
• Public Questionnaires/Questionnaire Inbox
• Metrics and Reporting
• Integration Points

• Define any type of question (Y/N, Numeric, Multiple Choice, Text, Date or Table look up) that can be added to a questionnaire
• Classify the questions into categories for easy reference
• Create a question library so that you can add the same question to many questionnaires
• Add headers/footers, company logos and default answers to a questionnaire
• Attach images to any question within a questionnaire
• Create complex formulas inside questionnaires to support/simulate various environmental assessment tools
• Attach questionnaires to any record in the Medgate system
• Build intelligence into your system by configuring findings & actions to be generated from certain question responses
• Link questions to environmental legal requirements (see the Compliance Management module)
• Deliver questionnaires to employees by printing, exporting into excel and e-mailing, or sending a standalone link for online submission

• Send employees a standalone link to respond to a questionnaire and that is submitted directly to the Medgdate system (they will not have access to any other record in the system)
• View all questionnaire responses for questionnaires you have access to from your questionnaire inbox
• Before posting the questionnaire, you can review and make changes as necessary

• Analyze responses by either the source questionnaire or individual question, with the option to graph the data
• Insert complex formulas into your questionnaires so that you can perform sub-section scoring
• Manipulate/graph questionnaire data using Medgate’s Business Intelligence tools

The Questionnaire module is an integral component of all of the Medgate Software Suites. It integrates with many modules in each suite.
