Jay-R-Smith - Model FormIt 3001-3009 -Square Floor Sink with Drain Installation Device
This 12" (305 mm) square floor sink was developed to save contractor installation time and speed construction. The assembly comprises the cast iron sink, acid-resistant coating, a removable foam form, and a carriage. This sink is topped with a nickel bronze cover and protects drain lines with its aluminum dome bottom strainer.

Secured by four screws, this FormIt® sink assembly is screwed to decking so it doesn`t shift when concrete is poured. Once the concrete sets, the foam is easily removed leaving an open path for connection to the waste line. The need to reset misaligned drains is eliminated.
